A Cross-Sectional Study of Blood Collection, Component Preparation, and Utilization Pattern in a Tertiary Care Hospital Blood Bank.

Aim
The paper represents an exhaustive study of the administration of blood banks in a tertiary care teaching hospital over a period of three years (2019-2021).
Method
There has been a total of 32,767 units of blood collected with community donation camps accounting for as much as 92% of the requirement. This represents the significance of organizing campaigns among the local populace.
Results
The demographic characteristics of the donor have revealed that most of the donors were males (94.56%) in the age group of 18-30 years (59.11%), reflecting the awareness among the youth and gender inequality simultaneously. The preparation of components has been quite effective, producing 60,524 units of PRBCs as the predominant one (39.8%). The maximum usage of blood was observed in the field of medicine (32.69%), followed by obstetrics and gynecology (20.7%) and surgery (17.37%). There was minimal wastage of blood, varying from 1.88% to 3.89%. Major wastage of blood occurred because of hepatitis B surface antigen positivity. Utilization indices such as cross match to transfusion ratio (1.05 to 1.19) and transfusion index (0.83 to 0.94) reflect rational transfusions in our setup.
Conclusion
High utilization of voluntary donors, use of components instead of whole blood, and efficient utilization of blood all signify good practices of transfusion medicine. We suggest therefore that there should be a continuous audit of the activities of the blood bank to minimize the wastage of blood.
Recommendations
To improve the transfusion services, it is suggested that more volunteer donors, especially women, be recruited through awareness programs and management of anemia. Routine audits should be done on the utilization indices so as to ensure the rational use of transfusions. The coordination among the different clinical units and the blood bank and better storage and handling practices will further minimize wastage.
